This is the first form for your family’s Family Dedication registration. Please complete all of the following family sections on this form.

Please complete the following sections once for your family:

If you are registering another child from the same family for the same Family Dedication, you may skip these family sections. However, please enter the same Father's Full Name in the Parent Information section so that we can identify the forms as belonging to the same family.
如果您要為同一家庭中另一位子女報名同一次家庭委身禮，可以跳過以上家庭資料部分。但請在「家長信息」中填寫相同的父親姓名，以便我們確認這些表格屬於同一個家庭。

Please complete the following sections for each child who will participate in Family Dedication:

If more than one child in your family will participate, please submit a separate form for each child.

Example / 範例：
If three children from your family will participate, please submit three forms—one for each child. On the first form, complete all the family information. On the additional forms, you may skip the family sections, but please enter the same Father's Full Name so we can connect the forms to the same family.

Please complete this section for your first child’s registration. If you are registering an additional child from the same family, you may skip this section, but please enter the same Father's Full Name below so we can identify the forms as belonging to the same family.

Please list all children, including their birthdays and whether they have participated in Family Dedication before.

4. Preferred Family Dedication Dates 家庭委身禮首選日子
To help create a meaningful and well-organized experience for each family, space for Family Dedication on each special Sunday may be limited.
為了讓每個家庭都能有更完善且有意義的安排，每個特別主日的家庭委身禮名額可能有限。
We encourage interested families to register early and select all dates that work for your family to allow for greater flexibility in scheduling.
鼓勵有意參加的家庭儘早報名，並勾選所有您家庭可以參加的日期，以便教會有更大的安排彈性。
Please note: The registration deadline for each Family Dedication date is one month prior to the dedication date.

The photos you submit here may also be used to update your child's profile in our church database (Breeze, used for checking into Children's Ministry programs), and your family photo may be used for our church's Photo Prayer Guide.
